Elizabeth Noar Physiotherapy, also known as North Berwick Physiotherapy, (‘we’, ‘us’, ‘our’) is registered to process personal and sensitive information under the Data Protection Act 1988. Our registration number is ZA069426. We are committed to respecting your privacy and to complying with applicable data protection and privacy laws.

If you do submit personal information by completing a web enquiry form, for example, you can be assured that we will use your personal information only to support your relationship with Elizabeth Noar Physiotherapy.

We hold information about patients, their medical conditions and clinical care. This information is held in our paper health records and electronically on our computer system. All information is held in accordance with the Principles of the Data Protection Act 1988 and all Practice have a legal duty to maintain your confidentiality.

We may share information with NHS or other healthcare providers and with your insurance provider (if applicable) for the purpose of ongoing care or treatment.

We also hold information about suppliers, business contacts and professional advisers.

YOUR RIGHTS

You have the right to:

  • Confidentiality under the Data Protection Act 1998, the Human Rights Act of 1988 and the common law of confidentiality;

  • Ask for a copy of all records about you;

  • Have errors in your details corrected

 

You can help us by:

  • Pointing out any information which is wrong and telling us when you change address, telephone number or GP;

  • By allowing us to share as much information about you as we need in order to provide you with the best possible healthcare.

 

What information do we hold?

We hold basic details about you, such as your name, title, address, telephone numbers, email and postal addresses, date of birth, and your GP, along with details of appointments and any financial transactions we have with you eg in payment of treatment received.

We hold other information about you that may include:

  • Your medical conditions (physical and mental health), and any medications prescribed;

  • Treatment provided and contact you have had with us;

  • Results of investigations such as x-rays, scans and laboratory tests that you have provided to us;

  • Reports about your health and the care you need;

  • Relevant information from other health care professionals;

  • Occupation, family details, lifestyle and social circumstances, financial details;

  • Exercise plans

 

Why do we hold this information?

Records about you are used by Elizabeth Noar Physiotherapy to:

  • Provide a good basis for all healthcare decisions by you and healthcare professionals;

  • Enable you to work in partnership with those providing healthcare;

  • Make sure the care we give is safe and effective;

  • Work effectively with others providing you with care;

  • Remind you about appointments;

  • Manage and administer your relationship with us (in terms of processing invoices, acknowledging payments, debt recovery and other administrative requirements);

  • Obtain customer satisfaction feedback for audit purposes through our patient evaluation survey; (We do not use your details for direct marketing or provide them to other parties for this purpose.)

  • Respond to complaints.

 

Who do we share information with?

We will share information with the NHS and other private healthcare providers in order to support your healthcare needs.

We will inform your GP of your progress unless you ask us not to.

We will also share your information with other organisations where it may benefit you, for examples community services or social services.

We will also share your information with your insurance provider if applicable, including any recommendations about work fitness if required, for the purpose of progressing your care.

Whenever we share information with other organisations we will do so in line with the Data Protection Act (1988). It may sometimes be necessary to transfer personal information overseas.  When this is needed information is only shared within the European Economic Area (EEA).  Any transfer made will be in full compliance with all aspect of data protection legislation.

 

How do we maintain our records?

Your health records are held in both paper and computer forms and we must retain these for at least eight years. Deceased patient information is retained for 3 years after death and for those under the age of 16, information is retained until the age of 25 is reached. After these retention periods, files are destroyed but details of name, date of birth and date of last treatment are retained indefinitely.

We have a duty to:

  • Maintain full and accurate records of the care we provide to you

  • Keep records about you secure and confidential

  • Provide information to you in a format that is accessible

 

How can you access you Physiotherapy records?

If you require a paper copy of your physiotherapy records please write to us to request a copy.

 

Social media, blogs, reviews

Any social media posts or comments you send to us (on the Elizabeth Noar Physiotherapy Facebook page, for instance) will be shared under the terms of the relevant social media platform (e.g. Facebook / Twitter) on which they are written and could be made public.

Other people, not us, control these platforms. We are not responsible for this kind of sharing. We recommend you should review the terms and conditions and privacy policies of the social media platforms you use. That way, you will understand how they will use your information, what information relating to you they will place in the public domain, and how you can stop them from doing so if you are unhappy about it.

Any blog, review or other posts or comments you make about us, our products and services on any of our blogs, reviews or user community services will be shared with all other members of that service and the public at large. Any comments you make on these services and on social media in general must be not offensive, insulting or defamatory. You are responsible for ensuring that any comments you make comply with any relevant policy on acceptable use of those services.

Elizabeth Noar Physiotherapy recognises that its patients are increasingly concerned about how companies protect personal information from misuse and abuse and about privacy in general. We are constantly reviewing and enhancing our technical, physical and managerial procedures and rules to protect your personal data from unauthorised access, accidental loss and/or destruction.

Please be aware that communications over the Internet, such as emails/webmails, are not secure unless they have been encrypted. Your communications may route through a number of countries before being delivered – this is the nature of the World Wide Web/Internet. We cannot accept responsibility for any unauthorised access or loss of personal information that is beyond our control.

Our website may contain links to other websites of interest. However, once you have used these links to leave our site, you should note that we do not have any control over that other website. You should look at the privacy statement applicable to the website in question.

 

Privacy Support

Elizabeth Noar Physiotherapy reserves the right to amend or modify this Privacy Policy Statement at any time and in response to changes in applicable data protection and privacy legislation.

If we decide to change our Privacy Policy, we will post the changes on our website so you know what information we collect and how we use it. If at any point we decide to use personally identifiable information in a manner different from that stated at the time it was collected, we will tell you. You will have a choice as to whether or not we are able to use your information in this different manner.
